Complaint Academic/Instructional Grievance

An Instructional Grievance occurs when a student feels they have been treated unfairly or unjustly by a full-time or adjunct faculty member regarding an academic process, such as grading, testing, or assignments. This form allows you to address an Instructional Grievance at STEP 3 with the Vice President of Academic Affairs or the Dean of Academic Affairs. Please ensure STEP 1 (you have discussed the issue with your Instructor) and STEP 2 (you have filed a written complaint to the Faculty/Department Chair) have been followed before completing this report. See the Academic/Instructional Grievance Process for detailed information on the process and the steps to follow.
