Required disability documentation
At Chandler-Gilbert, our goal is to provide full support to students with disabilities in compliance with federal and state law. The ADA, in particular, requires specific documentation for different types of disabilities, as listed below. Orthopedic, Blind/Visual Impairment, Deaf/Hearing impairment, Other Health Impairments
Medical report stating extent of injury or illness
Specific Learning Disability:
Reading, Writing and Math
Psycho-educational evaluation by qualified professionals
Attention Deficit Hyperactivity Disorder, or Attention Deficit Disorder
Depression, Anxiety, Bipolar, PTSD etc.
Diagnostic reports, current psychological evaluation, list of functional limitations
Traumatic Brain Injury, Stroke, Seizure
Temporary Disabling Condition/Impairments:
Surgery, accident, serious illness
