Find the right form for you

When you’re ready to apply for the educational benefits you’re entitled to, simply fill out and submit the correct paperwork from the lists below.

  • Request for Benefits Form - This form must be completed each semester.
  • Promissory Note - Form is initiated by the Veterans Services office once the Request for Benefits form is received. Will be electronically routed to student’s Maricopa email address for completion. 

  • If you are taking classes at another institution and CGCC is your parent/home school, you will need to complete the Parent School Letter Request Form. This form will allow other MCCCD Colleges to certify courses towards your degree at CGCC.
  • If you want to take a course with CGCC as your guest school, you will need to submit a Parent School Letter Request Form and a Request for Benefits Form.
  • Promissory Note - Form is initiated by the Veterans Services office once the Request for Benefits form is received. Will be electronically routed to the student's Maricopa email address for completion.
