Once you have been awarded financial aid for your college education at CGCC, it’s your responsibility to maintain your eligibility and make sure you’re following up as needed. Sometimes special circumstances arise — such as appealing a financial aid denial, requesting a student loan, or other situation — and you’ll need special forms for those times. Below are the forms you may need for 2020-2021.

Use this form to upload any documents requested from the financial aid office that do not have their own specific link.
Consortium Agreement 2020-2021
This form is needed when you are taking classes at CGCC and another Maricopa Community College, Arizona State University or Central Arizona University and you would like to receive your aid based on those credit hours. Before submitting the agreement to the financial aid office you must enroll at both CGCC and the participating school. There are deadline dates each semester to submit a consortium agreement. Please check with the CGCC Important Financial Aid Dates section for these dates.
Satisfactory Academic Progress Appeal 2020-2021
You’ll submit this form if you have been denied financial aid due to your academic progress and you wish to appeal.
You might need this form when you want to change your enrollment status, cancel your financial aid, request a student loan, or make other requests.
Maximum Time Frame Appeal 2020-2021
You are expected to complete your academic objective in an allowable time frame. Once you have reached this timeframe (for example, you’ve already completed a degree or have attempted excessive credit hours), you’ll need to complete this form for the Financial Aid Office to determine if you’re eligible to receive financial aid at CGCC.
Direct PLUS Loan Request Statement 2020-2021
You must complete and submit this form for a Direct PLUS Loan.
