Financial Aid Forms


Once you have been awarded financial aid for your college education at CGCC, it’s your responsibility to maintain your eligibility and make sure you’re following up as needed. Sometimes special circumstances arise — such as appealing a financial aid denial, requesting a student loan, or other situation — and you’ll need special forms for those times. Below are the forms you may need for 2021-2022.


General Document Upload
Use this form to upload any documents requested from the financial aid office that do not have their own specific link.

Consortium Agreement
This form is needed when you are taking classes at CGCC and another Maricopa Community College, Arizona State University or Central Arizona University and you would like to receive your aid based on those credit hours. Before submitting the agreement to the financial aid office you must enroll at both CGCC and the participating school. There are deadline dates each semester to submit a consortium agreement. Please check with the CGCC Important Financial Aid Dates section for these dates.

Satisfactory Academic Progress Appeal
Log into the Maricopa Counseling Center below with your MEID and Password to complete the appropriate tutorial and access the appeal. If this is your first time logging in, please click below to register a new account.
You’ll submit this form if you have been denied financial aid due to your academic progress and you wish to appeal.

Maximum Time Frame Appeal

You are expected to complete your academic objective in an allowable time frame. Once you have reached this timeframe (for example, you’ve already completed a degree or have attempted excessive credit hours), you’ll need to complete this form for the Financial Aid Office to determine if you’re eligible to receive financial aid at CGCC.

You must complete and submit this form for a Direct PLUS Loan.

Program Change Exception Appeal
Students may self-serve program changes until the first day of the semester. Following the official start date of the semester, any program changes must be made by the Admissions and Records Office and may only be done until the registration deadline for Pell Grants (Pell Recalculation Date or PRD) for the term. After PRD, any changes to a student’s program of study will be effective for the following term. If a student has made a good faith effort to change their program of study and was unable to do so before the PRD deadline, they may submit this form to have their appeal reviewed. If the appeal is approved, the program change will be effective for the current term.
